Care Coordination Services

Care Coordination

At Well Life Services we believe that excellent services follow an in-depth consultation process. In this way we’re able to tailor our supports for each unique individual.

Our experienced and dedicated team of Care Coordinators listen to and work with individuals and their families to develop relationships that promote self-direction in order to support independence and the achievement of personal goals.

We aim to provide high quality supports ensuring that your individual needs and preferences are known and communicated at the right time to the right people and that this information is used to guide the delivery of safe, appropriate and effective supports.

Our amazing team of Care Coordinators strive to help you establish a life of opportunity by identifying and providing a team of Lifestyle Support Workers who will support and encourage you to realise your goals.

We believe that people thrive when they have every opportunity to be a part of their community; to contribute and be involved by developing and supporting new relationships whilst maintaining existing relationships.

Additionally, our Care Coordinators have an excellent reputation for providing complex care for participants with Acquired Brain Injury (ABI) and complex health care needs following a traumatic injury.

BROAD CARE COORDINATION

NIISQ Attendant Care and Support

Well Life Services has been providing Attendant Care to individuals residing in South East Queensland since its inception.

Our Lifestyle Support Workers provide care and assistance for people with living with disability to perform tasks they would normally be doing for themselves. These workers provide assistance for the functional needs of a participant who has a physical disability and may need assistance with activities of daily living. Our services also include the performance of delegated nursing functions.

Attendant care services focus on maximising our participant’s independence across a variety of settings.

Activities of daily living include:

  • Bathing.
  • Toileting (Bladder and bowel requirements, movement to and from the bathroom).
  • Mobility transferring from a bed, chair or other structure and moving about indoors and outdoors.
  • Moving, turning and positioning the body while in bed or in a wheelchair.
  • Eating, nutritional planning, and preparing meals including prescribed special diets.
  • Dressing and changing clothes.
  • Taking medications or other remedies that are ordinarily self – administered, when ordered for the participant by a physician.
